NHIT DATA FUSION CENTER®

The NHIT Data Fusion Center, a collaboration between NHIT, AWS, and Tyler Technologies, revolutionizes data utilization. The DFC offers intricate insights into Social Determinants of Health (SDoH) by aggregating de-identified, publicly available data and tools. With support from Amazon Web Services, including credits for NHIT’s Care Campaign in Puerto Rico and the USVI, and the development of the NHIT Data Fusion, this partnership goes beyond transactions; it’s a strategic investment in community health.

NHIT INSPIRED

The NHIT INSPIRED AI Academy is a unique interdisciplinary program for dual-enrolled high schoolers, college and graduate students, and working professionals interested in using artificial intelligence to transform healthcare and advance health equity. Through real-world projects, mentorship, and professional development opportunities, participants gain hands-on experience addressing critical public health challenges such as food insecurity and chronic disease. They also have access to exclusive industry events like the HIMSS Global Health Conference and engage in roundtable discussions with leaders in health IT, policy, and innovation. By fostering cross-level collaboration and providing clear pathways into the digital health workforce, the Academy serves as a powerful launchpad for the next generation of changemakers.

USVI Community Health Empowerment

The NHIT pilot project in the U.S. Virgin Islands integrated community health outreach workers into a comprehensive, locally driven engagement strategy. By building trust and involving residents directly in the solution, the initiative strengthened public participation in health-related decision-making. It streamlined data collection through advanced tools and implemented asset and social health (ASH) mapping via a centralized NHIT Data Fusion Center. This approach enabled real-time insights into community needs and resource gaps. As a result, the project activated more sustainable and equitable access to healthcare services across the territory. Ultimately, the initiative empowered communities, enhanced health data infrastructure, and laid the groundwork for long-term improvements in population health outcomes.

Rural Georgia Pilot Project

NHIT partnered with Fort Valley State University to launch a demonstration project aimed at improving diabetes management in historically marginalized communities in Georgia. The initiative combines digital health tools with targeted community engagement to deliver better outcomes. By leveraging real-time data, AI-powered analytics, and mobile technologies, the project identifies high-risk populations, supports patient self-management, and informs resource allocation. Fort Valley State provides local insight and outreach capabilities, while NHIT drives the integration of innovative digital solutions. This collaboration is part of NHIT’s broader mission to reduce health disparities through tech-enabled, community-based interventions and serves as a scalable model for advancing chronic disease management.

CAPITAL BRIDGE

At the heart of this initiative is a dynamic, evolving funding toolkit, a resource shaped by community input and tailored to address the most pressing challenges entrepreneurs face. By integrating real-time insights, this toolkit will provide actionable guidance to help founders navigate funding landscapes, make informed decisions, and unlock capital to scale their solutions.
